Milestone HCQU West PCT Training

Person-Centered Thinking is essential to Person-Centered Planning. Those who provide support will learn how to help people with disabilities regain positive control of their lives. Trainees will learn skills that improve the lives of the people they serve and make their own work more effective and productive.

This training consists of 6 modules, each approximately 3 hours. Trainees must attend all 6 modules to receive credit.

This is an interactive training that requires participants to work in Zoom breakout rooms, either in groups or pairs.
** TO JOIN THIS TRAINING, YOU MUST **
- Have the Zoom app or access to Zoom.com
- Have a computer with a front-facing webcam - not a cell phone or tablet
- Have a built-in computer microphone or headphones with a microphone
- Have a strong internet connection
- Have the ability to print/access documents - see handout link below
- Use one of the following internet browsers: Chrome, Microsoft Edge, or Firefox
- Have a private or quiet workspace
- Have your own computer - we ask you not to share with others

Additional Info

After you register you will receive communication from PCSsupport@ReplacingRisk.com approximately two weeks prior to the training start date. This communication will include instructions and a survey that you will need to return. Once this is complete you will receive the following via email:
- A Zoom link to join for the training
- Handouts to download (these are required for this training)
- Instructions to follow on the day of the training
- Trainer information
- Instructions for cancellation or rescheduling

** PLEASE READ **
To receive a certificate for this PCT training, you MUST:
- Be engaged in the training.
- Be in attendance with your webcam on for at least 85% of the full training day. Please note that attendance will be tracked via Zoom Analytics.
- Be on time. If you join the webinar 15 minutes late or leave more than 15 minutes early, we will not be able to issue a Certificate of Attendance for your participation.

Trainer

David Toback MA

Person Centered Services Mentor

David has nearly 16 years of experience working in the field of Intellectual and Developmental Disabilities. He has worked both at the provider level and the state level as a training and project manager for GA Department of Behavioral Health and Developmental David has nearly 20 years of experience working in the field of Intellectual and Developmental Disabilities. He has worked at both the provider and state levels as a training and project manager for the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). David has experience in transition initiatives for those moving from institutional to community settings and has also worked as a Supports Intensity Scale (SIS) interviewer for many years. He is a commissioned Mentor Trainer in Person-Centered Practices by The Learning Community for Person-Centered Practices. He is also a Charting the Life Course Ambassador through Charting the Life Course Nexus and a PATH & MAPS facilitator. He joined IntellectAbility in February 2016.
